Photos and videos provide North Shore Church a means to promote and share information for our events and ministry activity.    

However not everyone appreciates being photographed or having their personal images or those of their family stored and utilized to prepare communication material that may be broadly shared within our communication channels [Facebook, NSC Website, Emails, etc.] For this reason we ask all individuals and their family members whose image may be captured and stored by North Shore Church to sign this form expressing their willingness and understanding that North Shore Church will store and use these images.

By signing this form, I give North Shore Church of Menominee Michigan permission to photograph myself and my family and store and use these pictures in the church’s digital and print communications.  North Shore Church will never publish a minor child’s name or any other personal information with any of its publications.
